Athletic Forms

Student-athletes, in compliance with the league office, ICA Cristo Rey will now be using the platform  HomeCampus.com to complete required forms. Hard copies will no longer be accepted.

Process for athletic eligibility

Step 1: Complete the digital clearance packet

• Create an account or log in here.
• Complete, sign, and upload the 7 required forms.

Step 2: Athletic Director will review the information and clear students for participation.
